• BLS - Basic Life Support
• ACLS - Advanced Cardiac Life Support
• PALS - Pediatric Advanced Life Support
• NRP - Neonatal Resuscitation Procedure
• Certifications as applicable
  • Job Details
  • Required: At least 2 years of care management with discharge planning experience in a hospital setting
  • Required: CarePort (WellSky) and EPIC experience
  • Our Case Managers are service based, they follow provider teams. Coverage would be for any of our adult teams.
  • Case Management Assistants help with CarePort referrals.
  • Ratios: 1:28.
  • Our RN CMs are service based and we utilize our full-time floats in these assignments as needed when there are vacations, call-offs, PTO, and other scheduled days off.
    • Job Requirements
    • At least 2 years of care management with discharge planning experience in a hospital setting
    • Experience with CarePort (WellSky) and EPIC
    • BLS Certification Required
      • Schedule Information

      • No weekends or holidays required
